Community First Services and Supports (CFSS) is a flexible, person-centered Minnesota Medicaid program that helps individuals with disabilities or chronic health needs live safely and independently at home. CFSS offers assistance with daily tasks, health-related needs, and support tools like emergency response systems and home modifications.

Community First Services and Supports (CFSS) is a Minnesota Medicaid program designed to assist individuals with disabilities, chronic health conditions, or age-related needs in maintaining independence within their homes and communities. CFSS replaces the Personal Care Assistance (PCA) and Consumer Support Grant (CSG) programs, offering a more flexible and person-centered approach to care.
Services Covered by CFSS
CFSS provides a range of services tailored to individual needs, including:
- Assistance with Activities of Daily Living (ADLs): Support with essential daily tasks such as bathing, dressing, grooming, eating, mobility, and transferring.
- Instrumental Activities of Daily Living (IADLs): Help with tasks like shopping, meal preparation, laundry, and managing medications.
- Health-Related Tasks: Assistance with medical tasks delegated by a licensed professional, such as administering medications or managing medical equipment.
- Behavioral Support: Observation and redirection of behavior as outlined in the individual’s care plan
- Skill Acquisition: Support in developing, maintaining, or improving skills needed for daily living and independence.
- Assistive Technology and Environmental Modifications: Provision of devices or home modifications to reduce the need for human assistance and enhance safety and independence
- Personal Emergency Response Systems (PERS): Access to devices that allow individuals to call for help in emergencies, ensuring safety and prompt assistance when needed.
These services are designed to be flexible and responsive to the unique needs of each individual, promoting greater autonomy and community integration.
